Health is affected by more than medical care. Access to nutritious food, stable housing, transportation, utilities, and community support can all affect a person’s health and well-being. Through New York’s Social Care Network program, eligible Medicaid members can be screened for health-related social needs and connected with appropriate resources.
New York’s Social Care Networks coordinate screening, navigation, and qualifying health-related social-need services for Medicaid members. All Medicaid members may receive screening and navigation, while enhanced services require additional eligibility criteria. (New York State Department of Health)

Who Can Receive a Screening?
New York Medicaid members can receive a health-related social-needs screening and navigation to existing community resources.
Enhanced Social Care Network services have additional Medicaid, population, clinical, and social eligibility requirements. Completing a screening does not guarantee approval for a specific service.
This distinction is important because screening and navigation are broadly available to Medicaid members, while enhanced services are limited to members who meet program requirements. (New York State Department of Health)
